Go to the Mission Control website
http://www.users.globalnet.co.uk/~hwind/up_.htm

click on "control"   ((this is one of the little menues in the red eye -
right bottom hand corner))

then click on "passports"

here you will find instructions and two files that you can download and
print from your computer as well as the address to send in the application.
Its all there.
So easy even I got one :-)
